The place Shuttle Challenger exploded mulctly after lift-off on January 28, 1986. Launch had been scheduled for January 22. It was delayed to the 28th following a series of problems. The NASA managers became sorticularly eager to get the mission underway. On the 27th, an engineer signalled a red flag, refer basket to concerns he and others had regarding the O-rings in the lifter rockets. Several conference calls were held to discuss the issue before a decision was made to go ahead with the launch. O ring failure might have been seen to be a reasonableness of the disaster. The decreed enquiry attributed it to human failure.
Under force per unit ara, the NASA managers had contradicted the goals that they had set themselves. For them as a group, this itself turned out to be a problem. The official enquiry also cited the teams feeling of invulnerability, the pressure from Congress and the group think pattern of social concord in which they failed to properly examine the risks attaching to their decisions.
While this is a striking and tragic example of failure attributable to human and organisational causes, these factors sack up also impact projects positively.
People and their organisations are the theatre of decisions: they are complex and will always leverage the course of a project or programme. This reality has to be addressed skilfully by leaders when sharing responsibility for projects and their outcome.
